Religion, Spirituality, and Health Status in Geriatric Outpatients Essay
Daaleman, Perrera and Studenski wished to re-examine the effect of religiosity and spirituality on perceptions of older persons, operationalized as geriatric outpatients. The authors proceeded from two conceptual constructs.à The first is that self-reported health status is central to aging research.à The old know whereof they speak.à Self-ratings are valid because they correlate well with health status over time and, consequently, health service utilization.à The second construct is that, no matter how morally they lived as young adults, those in late middle age come to embrace religion and spirituality with more fervor. Prior research had scrutinized the relationship between religion and health perceptions.à Some results were inconclusive, an outcome that the authors attributed to failure to control for such covariates as spirituality. Definitions vary, the authors acknowledged, but they proposed defining ââ¬Å"religiosityâ⬠as principally revolving on organized faith while ââ¬Å"spiritualityâ⬠has more to do with giving humans ââ¬Å"meaning, purpose, or power either from within or from a transcendent source.â⬠à In turn, the dependent variable was measured by a single-item global health from the Years of Healthy Life (YOHL) scale, a self-assessment of general health (would you say your health in general is â⬠¦) and a 5-item Likert response from excellent to poor. Fieldwork consisted of including a 5-item measure of religiosity15 and a 12-item spirituality instrument in a 36-month health service utilization, health status, and functional status study among 492 outpatients of a VA and HMO network, all residents of the Kansas City metropolitan area. In most DoS attacks, the attackers flood the servers with forged requests and make servers delayed. 1.4 Types and Generation of DoS Attacks Generally, there are three major classifications of DoS attacks depending on the victims targeted by attackersââ¬âusers, hosts or networks though there are several types of DoS attack prevalent on Internet. US Cert advisory suggests that the three main types of DoS attacks are bandwidth, protocol and software vulnerability attacks. The major aspects that most DoS attacks are focusing on are bandwidth, CPU time and memory. Most common DoS attacks can be summarized as the following. 1.4.1 TCP SYN Flood Attack Flood type attacks are the first known form of a DoS attack and their attack ing mechanism of is quite simple ââ¬â attackers send more traffic to a server than it can handle. (Georgieva, 2009) SYN Flood attack is a protocol type and exploits the weakness of TCP/IP protocol. US CERT advisory defines SYN flood as ââ¬Å"an asymmetric resource starvation attack in which the attacker floods the victim with TCP SYN packets and the victim allocates resources to accept perceived incoming connectionsâ⬠. In TCP SYN flood attack, the legitimate users are ignored when the attacker initiates a TCP connection to the serve with a SYN. The victim server responds to the request with spoofed IP address and waits for ACK from the client side. Then, the connection table of the server is filled up and it neglects all new connections from legitimate users. This phenomenon can be clarified using Figure 1.2. Raw materials, in the manufacturing process, were transformed to packaged pasta on fully-automated 120 meter long production lines. The plants were specialized by the type of pasta they would produce, with the primary distinction based on the composition of the pasta, e.g. dry or fresh pasta, pasta with or without eggs and spinach. Also, even within the same family of pasta products, individual products were assigned to plants based on the size and shape of the pasta. From the CDCââ¬â¢s approximately 65% of the dry products went to the supermarkets, 70% of these (65% of dry products) went to super market chains, whereas, the remaining 30% went to independent super markets. The remaining 35% of dry products were distributed from the CDCââ¬â¢s to Barillaââ¬â¢s internally owned regional warehouses, which then distributed them to smallà independent shops ââ¬â Signora Maria Shops. Dry products destined for supermarket chains were distributed from the CDC to the chainââ¬â¢s own distribution organization, known as Grande Distribuzione (GD). While those destined for independent supermarkets were distributed from the CDC to a distributor known as Distribuzione Organizzata (DO), which acted as a centralized buying organization for a large number of independent supermarkets. The CDCs held a monthââ¬â¢s inventory for dry products, and 3 days for fresh products. Brando Vitali suggested the implementation of a Just-In-Time Distributionà (JITD) strategy, which is essentially the Vendor Management Inventory (VMI) strategy. Barilla will be in charge of the channel between the CDCs and the distributor and decide on the timing and size of shipments to its distributors. Thus, unlike traditional supply chains in which distributors place orders and manufacturers try to satisfy these orders as much as possible, in JITD Barillas own logistics organization would specify the appropriate delivery quantities ââ¬â those that will more effectively meet the end customerââ¬â¢s needs yet would also more evenly distribute the workload on Barillaââ¬â¢s manufacturing and logistics system. If implemented, Barilla can make better delivery decisions and improve its demand forecasts, be more effective in meeting end-customers needs, and more evenly distribute the workload on its manufacturing and logistics systems. Also, the inventory levels at CDCs will a lso be reduced. What conflicts or barriers internal to Barilla does the JITD program create? What causes these conflicts? How should Giorgio Maggiali deal with these internal conflicts? The main resistance internal to Barilla was from the sales and marketing functions, which Barilla, until now, has relied upon for its success. The sales representatives feared reduction in both their responsibilities and bonuses due to a flatted sales level. The marketing people also feared a reduction in responsibilities as trade promotions would be difficult to run with a JITD strategy. There were also concerns about inability to adjust shipments quickly to stock outs, lack of infrastructure to handle JITD, vague cost benefits, and increased competitor shelf space at distributors. I think Maggiali should demonstrate that JITD benefits not just Barilla, but also the distributors. He should run experiments at one or more distributor sites and prove his case. Also, Maggiali should encourage the marketing and sales people to look at the overall benefit to the supply chain. By getting the top management involved, by effectively advocating the benefits for the entire supply chain, and by removing the obstacles of sales incentives and reduced responsibility, Maggiali can effectively deal with this problem and get JITD implemented. How do you think a typical Barilla customer would respond to JITD? Why? How would you convince the customer that the JITD program was worth trying? If you are not able to sway the customer, what alternatives would you suggest to combat some of the difficulties that Barillaââ¬â¢s operating system faces? Literature on the Caste system in India There is a vast literature on Caste system in India with a long and diverse background. This chapter aims to review some of the relevant literatures pertaining to the caste system prevailing in India. Different authors might have varied perceptions about this particular topic for discussion. According to Louis Dumont (1980) in his book Homo Hierarchicus which has details about the caste system and its implication, demonstrates that Indian society was structured on a firm notion of hierarchy that was based on the relationship between the pure and the impure. It is more of a religious than a political or economic notion. He defines hierarchy as the principle by which the elements of a whole are ranked in relation to the whole, it being understood that in the majority of societies it is religion which provides the view of the whole, and that the ranking will thus be religious in nature(Dumont,L.1980). He also points out the prevalence of traditional hierarchy which was based on varnas or colours whereby people were divided into four categories namely Brahmins, Kshatriyas, Vaishyas and Sudras and; untouchables are outside the classification. M.N.Srinivas (1962) in his book Caste in Modern India and Other Essays, highlights the part played by caste in democratic processes of modern India in administration and education. The author came across certain conflicting attitudes among the people of the elite class whereby one group wanted legislation to eradicate the social evils pertaining to the caste system and on the other hand, there were people who were not only determined not to fight the evil but also tried to practise this system. In his work he tries to explain the concepts of two social processes namely Sanskritization and Westernization. Sankritization is the part of social mobility as well as the idiom in which mobility expresses itself. This is said to occur within the framework of caste whereas Westernization happens outside the framework of caste. However, Sanskritisation may lead to castes becoming unpopular with their neighbours whereby the leaders of upper or dominant caste may show their bitterness by even to rturing the members of the lower castes. In independent India, the reservations and safeguards granted to the backwards sections especially the Scheduled Caste and Tribes have helped in the upliftment of the lower caste. To the question of can castes exists in the India of tomorrow the author opines that only a minority considers caste as an evil to the whole nation and that this minority is gradually increasing every day. Taya.Zinkin (1962) in her book Caste Today describes the caste system in India. She considers its origin, the way it works, what democracy is doing to caste and vice versa. In her work she states that caste is not class and that every caste has educated and uneducated, rich and poor, well born and ordinary born. The author also says that caste is not dependent on colour because a Brahmin will not stop being a Brahmin if he is black skinned nor does an untouchable stop being one if he is fair skinned. She also argues that caste is not based on occupation, however various other literatures may not completely agree to what this author states. According to her caste is a way of life which divides society into small groups, each of which lives in a rather different way from the rest. Due to these differences, tiny groups and important aspects of life like marriage take place within them, these groups have immense control of power and thus a better survival. Before she goes into the detail s of castes, sub- castes and untouchability she tries to explain the concept of re- incarnation. It is said that the whole system is based upon a combination of status fixed by birth and rebirth. This means that a persons birth in the existing life depends on the consequences of his deeds done in past life i.e. if one performs his duties well complying with what he is supposed to do then he may be reborn in a better situation or not be reborn at all. Recent articles from The Economist which are mentioned above, mainly talks about the reservations and quotas based on caste rather than discriminating against them on the basis of ritual purity and occupation. However it is not completely true to say that caste system has vanished from the Indian society.
